Are you actually working on it for ten hours or are there numerous interruptions throughout your day.  Take a closer look at what actually occupies your time.

I would like to meet the MDS cord that can finish their raps in the time study < 75 min and a c/p as well. What am I doing wrong? My raps can take up to an average of 3hr if have known this person forever and they do not have any delirium, mood, depression, behavior, urinary inc., pressure ulcers and don't take psychotropic meds. I just completed a set of raps for a resident that took me est 10 hrs to complete/just raps, very complicated, behaviors, falls, wandering, no restraints, psychotropic meds, you name it............ PLEASE in put. My c/ps are so comprehensive as well, state has told me numerous times, if they can leave the residents name off the care plan, look and read the care plan, this should leave no doubt in their mind this is the resident. Everyone of my care plans have a golden rule and in nursing their is no such thing as always, but then rules are made to break? right? Skin care plan and Fall, automatic w/o ?
