It would be a restraint if it met the restraint definition
in the MDS book.  A question to ask is: If she is hitting
her head on the rail, should the rail be there, padded or
not?  What purpose does the rail serve and is there a risk
of entrapment?

Does anyone knows if 1/2 padded siderails for safety is
considered a 
restraint? Patient has dementia, very restless and has a
tendency to hit her head on 
the rail. Thank you.
