I'd include this info in her RAPS for now, and continue to assess her for 2weeks. If she is making progress and you feel that she is getting back to baseline, I'd just watch her to make sure that improvement continues. However, if she is not making any progress after the 2 week observation, a SIG change is indicated. Another option, is to change your A3a date if your time frame will allow and do the SIG change. However, if she later improves, you're gonna have to do another SIG change.

Just want everyone's opinion. I am currently in the process of completing RAPS on a resident who had a significant improvement due to many improvments with ADL's this assessment is replacing a already scheduled quarterly. Between the A3a date and R2B date the resident fell, banged her head has a subdural hematoma and has had a notable significant decline in numerous areas. Should I just include this information in my raps I'm doing now, or do another significant change assessment. I don't know whether she will improve again or not. She is a little better, but I doubt she'll be back to her prior level of functioning in 14 days. Thanks in advance
