I did a discharge return anticipated because we could no longer skill her with therapy(or anything else) and we were anticipating skilling later with therapy after weight bearing orders were changed. Could I combine the 2 assessments and do a 3/5?  Did you mean a 05 (medicare readmission/return) assessment or a 5 day (01) I already did an admission assessment with the initial admission 2 months ago. She has a new diagnosis but had approx 30days break so cant use new Med A period.
