"When a resident moves between certified and non-certified units within a facility,
appropriate tracking forms must be completed to document this movement, just
as if the resident had moved to a completely different facility."
The discharge tracking code Discharged Return Anticipated is used when a resident is temporarily discharged back to the hospital for an admission or a hospital observation stay of more than 24 hours.
Since the discharge was with return not anticipated, an Admission assessment (AA8a=1) is required on readmission by the end of day 14.
On readmission to the skilled bed and Medicare Part A, the correct PPS assessment is the Medicare 5-day AAb=1.
The Medicare/Readmission Return assessment is used only when a resident on Medicare Part A returns from a hospital stay on Medicare Part A. See page 2-31 of the RAI User's Manual.

I need help determining what type of assessment I should be doing.
This is the scenario
1/6/04 Discharged return expected from a medicare bed into a private pay (non certified) bed
1/27/04 Resident fell and broke hip and arm admitted to hospital
2/4/04 Back to nursing home â in medicare bed, continues with original med a stay I did a reentry
2/9/04 (I know it is day 6) now should I do scoc (meets the criteria and probably would even if I would wait 14 days?)
I need a comprehensive assessment â I usually do with 14 day but family is here now to care plan with so wanted to do it with this assessment
and should I also do a medicare readmission/return 03/05 hipps 35 or should I do
SSOC /5 day medicare (03/01) hipps 31
Or should I do something different?
I know what I would really like to do but it involves sun, sand, water, and an umbrella drink ; >
