I've had the same problem with answering Q2 (overall change in care needs in past 90 days or since last assessment) on an Admission/14-day since the 5-day MPAF DOES have that item.  So the 5-day is "2" (decline) and Admission/14-day is "0" (no change).  Is this what everyone else does?  It was always simpler to do the Admission with the 5-day because of this, but some 5-day assessments need to be finished shortly after the end of the month for Medicare billing so are not combined with the Admission Assessment due to time constraints.

Let me see if I understand.  You wouldn't count the 5 day MPAF as an assessment.  You would then code for hospital stay since there has been one in past 90 days.  Can I ask why you would not count the 5 day MPAF as an assessment?

The RAI Manual states on page 3-202

Intent:             To record how many times the resident was admitted to the hospital with an overnight stay in the last 90 days or since the last assessment if less than 90 days [regardless of payment status for these days either by the hospital or by the nursing facility].  If the resident is a new admission to the facility, this item includes admissions during the period prior to admission.

 

Since you are completing a 14Day/Admission assessment, I understand this to mean that you would count any hospitalizations prior to admission to your facility in the last 90 days.  I would not consider the 5Day MPAF assessment since it does not include this question.

Quick question.  On section P5 where it asks for hospital stays, this is not on the MPAF.  When you answer it on the admission/14 day do you check no, since there have not been any hospital stays since last assessment?
