We are case mix in Ohio. I do not do an annual to capture a higher score, but I do a quarterly early  when I know we have pneumonia, pressure, new therapy, anything that would raise my score. Rumor has it the state has been zeroing in on this, so I check to make sure it isn't three weeks later that I do another quarterly. At times this is a sig. change and end up doing a comprehensive, but not always, especially with pneumonia. Hope this helps.

Subject: multiple annual assessments

This question is for case mix states - recently had a consultant say that it was OK to do annual assessments every 3 months (not sig changes) instead of quarterlies in order to capture a higher case mix.  Does anyone actually do this? and what are your thoughts on this?
I'm also in Ohio, and yes, the state is zeroing in on this, per our corporate people.  However, the manual specifically states that quarterlies are to be done at a minimum of three.  So I don't know why the extra quarterlies would be considered a fraud or any other kind of problem unless you were attempting to capture the same time period (overlapped) just to raise a score.  As much extra work as the RAPS are, there is always the Sig Change for the worse, and then another for when they have improved.  I am still waiting for the state to show me the law in writing that says we can't do more than 3 quarterlies in a year. In the meantime, I have to follow the orders from corp. and not do them.  But it is killing our CMS.
