We have 8 units in our facility. Each shift is responsible for reviewing 1/3 of the care plans on their unit, along with the care card, and update them to reflect current cares delivered. At the time of the MDS and care plan, any changes made are reviewed by the care plan nurse and updated in the computer and care plans reprinted with changes made. This way at least all records are reviewed monthly. SBecker RN Trinity Homes
