We type up new careplans with every annual or SCSA. The
surveyors that we have had want to see that the care plan is a working care plan
and like to see the changes, updates, discontinuations of approaches/goals
etc. I agree sometimes they do get messy but that just means
you are doing your job...just my opinion though

Why do you "retype" care plans with every MDS? WHy not update the
care plan; i.e. goal date changed; resolved issues indicated and new
problems addressed.
Retyping care plans with every MDS generates ALOT of paper for the
chart.
