We always use yellow to d/c on a care plan. It is then dated and initialed, or signed indicating who d/c'd it and when, I also write "D/C". This is done in ink and can be copied. This kind of interference just makes me nuts. I think the surveyors get into areas that they have no business, and does not affect the quality of care delivered to residents. The only reason you would copy is either for the state, or a lawsuit. Or am I missing something?
