They are due by day 21 and each time an obra MDS is done[quarterly,annually,or with
sig change.The purpose of doing Obra MDS is to develop a care plan based on the
assessments of the MDS.Care plans are also updated as needed with changes in patients
condition,pressure,uti,pneumonia,on and on.The care plan shouldn't be a paper
compliance but a way for staff to deliver care.The RAI is specific with care plan
timeframes and the requirements.Surveyors expect to see an upto date careplan when
they visit,it must be reflective of what is presently happening to resident and
provide a current picture of the care being given.
