Careplans

Hi everyone, I was just wondering how many of you do carepan's by "problems" vs "raps".  I work with 3 separate units at our facility, Long Term Care, Sub Acute (Vents and Trachs) and also we do Rehab ie Medicare A.  I usually do Careplans based on what is triggered in the MDS and any problems that seem pertinent.  Our Sub Acute unit has their own careplans, but they are based on the raps, so careplan 1 will be "Delirium", careplan 2 would be "Cognitive Loss", of course you would only do careplans which are pertinent to the resident.   There are more careplans whch can be used as needed for other various problems.  The reason I bring this up as I would like to make new careplans which would be appropriate for all the units and it would be more consistent through out the facility.  I wish to simplify the system.  Any feed back would be appreciated if you can figure out what I wrote!!  :)   Mychal
