These are careplans written from the resident's perspective, using "I" instead of "the resident" which is third person.
 
For example a problem might read: "I tend to get dehydrated because: (insert reason here)".
 
An intervention might read: "I need you to remind me to drink my juice and water between meals".

We have been using them for 3 years…I think they take a little extra time to write…but the aides understand them and are more likely to read the first person narrative….the first survey we had, once we explained what we were doing, surveryor seemed to like them….you can email me personally if you have any questions.
