First person care plans read a bit like a letter
from the resident. Social services includes a paragraph about the resident’s
personal history as told by the resident.
(this is my favorite part…its amazing the
things we learn about our resident’s here ) . Goals are written like this…”My
goal is to (insert goal here) Help me reach my goal by (list
nursing approaches here).” Goals and approaches are typed in bold letters
and highlighted to draw attention to them. I use a template which includes all
areas we need to address…psychosocial, activities, personal care,
mobility, safety, nutrition, skin, etc. Resident’s that are able
participate in setting goals, families enjoy reading them, and the aides
understand what is expected of them in caring for the resident. First person care plans are highly
individualized (which surveyors like), the only down side is it does take
longer to write the initial care plan. -----Original Message----- What are

write…but the aides understand them and are more likely to read the first
person narrative….the first survey we had, once we explained what we were
doing, surveryor seemed to like them….you can email me personally if you
