First person care plans read a bit like a letter from the resident.  Social services includes a paragraph about the resident’s personal history as told by the resident.  (this is my favorite part…its amazing the things we learn about our resident’s here ) . Goals are written like this…”My goal is to (insert goal here)  Help me reach my goal by (list nursing approaches here).” Goals and approaches are typed in bold letters and highlighted to draw attention to them.
