return to facility

I need some help on a return to facility. We are a Skilled unit had a pt for over 30 days we sent to ER at 10  pm with resp distress the pt was transferred to OBS less that 24 hours sent back to us by 11am should we discharge return anticipated  and start over with a 5 day MDS are leave like it is and not change anything? Thanks Lisa
