If the patient was not in the hospital for over 24 hours or not admitted all you would need to do would be to subtract one day from their 100 days.  I would review the careplan to make sure it is still up to date and make a note explaining in hospital for 23 hour observation.  You would not need to do a 5 day/return/readmission as they were were not d/c'd.
