The manual lists situations where a d/c
tracking is not completed: When residents are in the hospital outpatient
department for an observational stay of less than 24 hours and the resident
is not admitted for acute care as an inpatient. I would not complete
d/c tracking form. I would however, upon residents return, update care plan
if needed, and document in the medical record. I put a separate
progress note in MDS section of all residents charts
so if needed I can document anything pertinent
to MDS/RAPs/care plan.

No head on the bed at midnight. Do a 5
day re-entry and restart the schedule.
