On QI #8, Prevalence of Bladder / Bowel Incontinence, High and Low risk are determined as follows.

High Risk: Severe cognitive impairment (B4=3 AND B2a=1) on the most recent assessment, OR Totally ADL dependent in mobility ADL's (G1 a, b, e-Box A self-performance = 4 in all areas) on the most recent assessment.

Low Risk: All others at the most recent assessment.

See this site for a detailed description of how each QI is calculated from the MDS:

Hope this helps!

Subject: QI on low risk and hi risk incontinence

Can anyone tell me what triggers a low risk or a high risk resident on incontinence. Where in the MDS does this pull from . Example some totally incontinent people trigger hi some low. Any input you have will be greatly appreaciated. Thanks in advance.
