On QI #8, Prevalence of Bladder / Bowel Incontinence, High and Low risk are determined as follows.   High Risk: Severe cognitive impairment (B4=3 AND B2a=1) on the most recent assessment, OR Totally ADL dependent in mobility ADL's (G1 a, b, e-Box A self-performance = 4 in all areas) on the most recent assessment.   Low Risk: All others at the most recent assessment.   See this site for a detailed description of how each QI is calculated from the MDS: http://www.chsra.wisc.edu/CHSRA/Quality_Indicators/toc.htm   Hope this helps!
