Numerator:
Residents who were frequently incontinent or incontinent on most recent assessment.
Bladder H1b= 3 or 4
Bowel H1a = 3 or 4
 
Denominator
All residents, except as noted in exclusion
 
Exclude:
Residents who are comatose or have indwelling catheter or have an ostomy at most recent assessment.
 
High Risk:
Severe cognitive impairment or totally ADL dependent in mobility ADLs (G1a,b, e-BoxA self-performance =4 in all areas) at most recent assessment.
 
Low Risk:
All others at most recent assessment.

Subject: QI on low risk and hi risk incontinence

Can anyone tell me what triggers a low risk or a high risk resident on incontinence. Where in the MDS does this pull from . Example some totally incontinent people trigger hi some low. Any input you have will be greatly appreaciated. Thanks in advance.
