Directions for QI are in "Pink" or "Watermelon" colored book.  Residents who are 
excluded are "Comatose (B1=1) or have indwelling catheter (H3d is checked) or have an 
ostomy (H3I is checked) at MOST RECENT ASSESSMENT.  
Risk adjustment for High risk is "Severe cognitive impairment, or totally ADL 
dependent in mobility ADL's (G1 a,b,e-box A self performan =4 in all areas.) at MOST 
RECENT ASSESSMENT
