Our format is a check off system, with a space for notes.  Because the questions are truly bizarre, they don't tend to lead to a good care plan, nor a full look at the problem.  I have a personal preference to look at the questions posed by the RAP in the manual for each area, and then make sure to notice that area in my RAP.  Tends to lead to a more rounded look at the problem, and to explain why I am or am not proceeding to care plan.
