I think what she was referring to are RAP review summary statements.  It is not really required to write a summary statement of the RAP review, only to note the source of the information on the RAP Summary page. I find that it is extremely helpful to compile all the info into one spot, with references to other sections included in the narrative. However, as always, YMMV.

I really need some help with Raps.  We do not do rap summaries  and i feel we should.  Could you give some idesa of what you do at your facility?  Does anyone have a rap sheet they are willing to share with me?

How can you not do rap summaries?  That is where you state the reason to proceed or not to proceed.  It is required.  This is what formulates the plan of care.  You are correct in thinking that you should be doing them.  I have samples of ones that I have done.  I could email them to you personally if you would like.  Let me know.
