Our LPN is excellent and she does everything that we do. But I sign off the R2b, Vb4, ect. She does however sign off on all the sections she does. As for care plans, she does initiate, update, ect. But techinically in the State of Ohio, LPN's are not allowed to initiate a care plan. I haven't seen it enforced yet. If it were, there would be a lot of nursing homes without any care plans.
