At our facility the Care Coordinator is an LPN; she completes the MDS and does all the care plans, including collecting data for them. She also conducts the care plan meetings with the facility staff and families present; but there is not always a RN present at these meetings. The LPN does a good job and we have never been cited for this action,however, in Pa., I do believe a RN is required to complete the care plan assessment and the care plan document itself or sign off as verification if a LPN does complete these duties.
