I'm sure these questions have been asked many times before...  Who does the UNSKILLED MDS's and Care Plans in your facility?  How many residents do you have?  Who has a helper that does some of the MDS's and care plans?  Does she do anything else?  Who goes to the care plan meetings?  What kind of problems are you having, if any?  We are thinking of changing some things at our facility to make everything go smoother. In our facility, we have 123 beds. The staff nurses are assigned room numbers that they are responsible for completing the MDS Assessments. I do all the scheduling of the assessments, check, lock, and transmit. I schedule the care plan dates, but do not attend the care plan meetings. These are attended by the other disciplines, and charge nurses. I run reports for our DON, including the Quality Indicator report each month. I am a member of the QA committee, Skin Team, Mentor Program, Creative Problem Solving team, Policy and Procedure, and I am a member of the Eden Alternative team and one neighborhood. We just revised all our care plans, and I headed that team. I review the documentation forms and revise them. I am in the process of streamlining our documentation to prevent duplicate documentation. I inservice the new GNA 's and CMA's on documentation requirements, ADLs etc. I inservice the nursing staff on the MDS process and train them how to accurately document. I do a monthly newsletter to the nursing staff pertaining to MDS issues. I do not have any other staff working with me in the MDS office.

The same nurses do the Medicare and the unskilled assessments.
