Our facility is 118 beds dually certified but average about 30+ medicare with average of 20+ admission per month. I complete all the medicare assessments (including OBRA while medicare is the payor-this includes required RAPs and care plans), do acute care plans, update all care plans per physician order changes as required, am over falls and restraints, attend care conferences, medicare meetings weekly and falls, restraints, standards of care, utilation review monthly. We are part of the Medical Review of NC pain and restriant program which I oversee for this facility. There is also an LPN who does all the medicaid assessments, RAPs and care plans. Currently I work around 50+ hours to stay current, but once the LPN graduates and can put in more hours, I plan to delegate some work to her. Davina DeMerritt, RN

I'm sure these questions have been asked many times before... Who does the UNSKILLED MDS's and Care Plans in your facility? How many residents do you have? Who has a helper that does some of the MDS's and care plans? Does she do anything else? Who goes to the care plan meetings? What kind of problems are you having, if any? We are thinking of changing some things at our facility to make everything go smoother. Thank you in advance. Karen
