There is not an absolutely fool proof system.  The common working file will give you basic information but is dependant on bills that have been filed.  There is no way for Medicare or anyone else to be accurate until all bills are submitted and entered into the system.  We get that information, get history from resident/family, call all appropriate facilities for information and piece it together as best we can.  We usually get it right but occasionally we are wrong and pay the price.  This occurs especially if someone comes from another area and there is no good historian available.

Our Case Manager/Discharge Planner needs to know how to easily find out if a pt that acute care wants to send to us has any remaining Medicare days and how many they might have.  Isn't there some way she can find this out?  Especially for pts who are also in and out of other Skilled Nursing Facilities in addition to our Acute Care hospital and attached Transitional Care Facility---and all of this may be part of the same illness with no healthy period in between.
