The accountability book is for the nurse assistant. It is a "cardex" that indicates the level of assistance the resident needs. Number for transfers, feeding, toileting, DNR, choking risk, bed mobility etc. it indicates special equipment also. There are sheets seperate from the "cardex" that they sign for purposes of ROM minutes and ambulation and meal intake, T&P, and toileting program.
