I do not code it unless they have ambulated (either alone or with assistance) during the lookback period.  If they are not ambulatory because of history of unsteady gait (for example, they now have a waist belt due to persistent falls) then I address that in the RAP and care plan, but don't code for it.   HS Holly F.
