This happens quite often with our team. While completing the quarterly we realize that there is a significant change. We then change the assessment to a significant change and use the same ARD date. We complete the RAPs within the week. If there is any doubt about if a significant change has occured we complete the quarterly as scheduled and evaluate the resident further for 14 days. If at the end of the 14 days the changes are still present, we proceed with a significan change assessment.
