We use Achieve Datacare Plus & there is 1 # that will allow you to change the date or reason but this is only if the assessment has not been closed.  Once it has been closed it must be deleted & then a new assessment, with the same ARD but different reason put in in.

Does your software allow you to change AA8b from a 5 to a 3? Do that, put in the additional data and you should be done.

This is probably a dumb question but what the heck.  While doing a quarterly MDS it is noted that resident has had improvement in several area's.  It is it necessary to do an entire new assessment or can I use info from quarterly open a sig change assessment and use that information?  Keeping the same ARD or do I need to use a new ARD?    Of course RAPs and careplan changes will occur.  Thanks,
