This is probably a dumb question but what the heck. While doing a quarterly
MDS it is noted that resident has had improvement in several area's. It is it
necessary to do an entire new assessment or can I use info from quarterly
open a sig change assessment and use that information? Keeping the same ARD or
do I need to use a new ARD? Of course RAPs and careplan changes will occur.
Thanks,
Michelle

Michelle,
This happens quite often with our team. While completing the quarterly we
realize that there is a significant change. We then change the assessment to a
significant change and use the same ARD date. We complete the RAPs within the
week. If there is any doubt about if a significant change has occured we
complete the quarterly as scheduled and evaluate the resident further for 14
days. If at the end of the 14 days the changes are still present, we proceed with
a significan change assessment.
