one way to make them do it even without computers to input is to make them write down in a sheet of paper their decision as to whether they want to proceed to care plan and why, why not. they do this after they completed their sections. then they submit it to you for review and then is typed in the computer. some softwares can print to you the triggered RAP and then there is a blank space for you to write the RAP review note. if you have such software, you can print this out for them then they can return it to you so the RAP process can be completed in the computer. as to POC, you have to coordinate with your DON so that the nurses can also participate in care plan completion even before the MDS is completed. it should not only be you who should be completing it. also, require each discipline to care plan on their particular triggered RAP. if they don't they should not be in the building because that means they do not know what to do with the residents and how to care for them - nutritionally, psychologically, psychosocially, emotionally etc...
