For quarterly assessments ... no more than 92 days from R2b to R2b.   Annual assessments ... no more than 366 days from VB2 to VB2.   Page 2-3 of the RAI manual has a Table  to refer to for completion timeframes   Care Plan Completion Date (VB4) must be dated by the end of the 7th calendar day following VB2 (VB2 + 7 days) and can be no later than 21 days following the ARD.   The regs do not address how long after a quarterly the care plan has to be updated or revised, just that it should be periodically updated to reflect the residents status. It only addresses after a comprhensive assessment.
