Just an addendum to my last post.  We do have a State law in CA for a Centralized Unit record.  However, if you follow the requirements in the RAI manual and write your policies accordingly you should not have problems with CA surveyors.  Key word is 'should'.  Remember you are responsible for knowing the regulations and requirements.  If you don't, the survey team can intimidate you into doing anything they say.

What state? The requirement is that the MDS be accessible. It does not specify in the chart. Many facilities have separate binders, which is not in conflict with the requirement. Obviously, the binders have to be in a secure location.

state also told us  MDS's must be kept in the charts.  A secretary had thinned a chart without my knowledge and had thinned out some MDS's into a file drawer next to the charts.  This was a no-no per surveyor.
