In Louisiana Surveyors will accept 15 months of MDSs in the binders under the following conditions:
1. MDSs are filed systematically (Example, by Hall, by room number OR by Hall in Alphabetic order, etc)
2. Can be stored as an extension of the active chart in binders labeled according to #1 above.
3. MUST be stored in readily accessible area - interepreted as to close proximity of active chart. Example: on top of active chart rack. NOT in the nurses' med room, chart room, MDS coordinator office, etc. Filing in these places WILL get your facility cited.
4. ALL nursing staff MUST know where the MDSs are located. You WILL get cited if surveyor ask a nurse where the MDSs are located and he/she can't tell.
5. As new staff members are hired, they must be acclimated to the whereabouts of the MDSs.
6. I recommend placing a typed paper in the chart behind the MDS tab stating "15 months of MDSs are located in the binders on the top of the active chart rack at the nurses station."
**This alleviates the risk of staff being asked where charts are located - because surveyors will know where they are located because you have communicated that information to ALL - consultants, departments, surveyors, etc.

These are kept secure in the nurses station report room along with policy and procedure manuals. Just have to be sure to thin MDS older than 15 months and move MDS with room changes. We keep the MDS admit face sheet in plastic page protector on top, then MDS from most recent to oldest.
