Also, Make sure you have a written policy outlining how you keep the MDS accessible and secure "to comply with state and federal requirements." and that everyone follows the policy.

 It's probably the "thinned without my knowledge" issue. That "no-no" was addressing the lack of a system that everyone followed.  Items truly aren't accessible if some have been removed and only one or a few staff know where they were moved to and how they are stored.

What state? The requirement is that the MDS be accessible. It does not specify in the chart. Many facilities have separate binders, which is not in conflict with the requirement. Obviously, the binders have to be in a secure location.

state also told us  MDS's must be kept in the charts.  A secretary had thinned a chart without my knowledge and had thinned out some MDS's into a file drawer next to the charts.  This was a no-no per surveyor.

None of our MDS are in the individual charts.  We have 6 big 3 ring binders organized with page dividers by room number (pharmacist prefers this).  These are kept secure in the nurses station report room along with policy and procedure manuals.  Just have to be sure to thin MDS older than 15 months and move MDS with room changes.  We keep the MDS admit face sheet in plastic page protector on top, then MDS from most recent to oldest.  System has worked well for 3 years.

State surveyor just left today and we got cited for keeping residents MDS in my office versus putting them in the charts.   We send the MDS to medical records for filing in the charts after pt. is discharged.  What are your thoughts on this issue?
