Just to clarify the HIPAA regulations. HIPAA does not require any authorization or release be signed for disclosure of information for treatment, Payment or Operations. The sharing of patient information for transfer is treament and is permitted under HIPAA (this is clearly stated in regulation).

Suggestion: get your medical director to sign a "required documents at transfer list" that includes medication sheets and Iv summaries, (and whatever else you want, H&P, op notes, lab etc) as well as a "release of info" statement for the patient/poa to sign. Then, whoever is responsible for the screening eval would get that signed by the client BEFORE transfer. This will suffice for the HIPPA standard, and with your medical director input, it has more weight with the hospital.

Does anyone have a D/C Summary sheet which includes IV fluid amounts received during a hospital stay. Our hospital will not release medical records to out off site facility due to HIPPA regs.
