I found a problem like that with Mac Plus using drives from external drive
shells. Turns out it was a wrong floppy drive cable.

It is worth noting that Apple used two types of 800KB drive mechanisms which require different ribbon cables -- one has a yellow tell-tale for pin 1, the other has the normal red tell-tale. Apple tech info article 1605 (amongst others) describes the differences.
