I've suffered similar symptoms on differing platforms.
Without seeing the OS X version and all things being equal, on your first boot Keep it Simple in your config. Aim to boot to a flashing "?" rather than have the emulation ready to share the lan, host file system and other devices.
Turned off the floppy option? ;)
JIT can cause problems in other ports.
Sheepnet or the BII ethernet drivers can cause all sorts of problems on first boot, turn networking off in the BII config!
Your not trying to boot a OS8.x image?
