Similar to http://www.cyberciti.biz/faq/centos-rhel-6-install-serial-console/
create /etc/init/ttyS0.conf:
start on runlevel [2345]
stop on runlevel [S016]
respawn
instance /dev/ttyS0
exec /usr/sbin/faxgetty ttyS0
and restart fixes problem.
