I saw this on a machine that was upgraded from 9.0 -> 9.1.  The weird
thing about it was that in KMixer, everything was up at 100%, but it
would play at very low volume.  Popped open aumix and Master was at 100%
but PCM was at 0%.  Set them both to about 70% and the sound came
booming out.

It seems in this case, that KMixer is the only thing that's screwed up.
Aumix works just fine with the stock configuration (at least on the
configuration I saw).  I can arrange to get system specs for anybody who
wants to compare notes.
