Here in germany are a lot of DSL users.They can download as much as they want and pay only their monthly fee (79 EUR most cases) and people don't have DSL know somebody who have it or go to an Iso-store.
Even though ADSL has individual links to the local exchange, which (shared) cable does not, popularity and the resulting congestion upstream will IMHO eventually force some kind of download capping rather than doing the necessary major exchange and trunk cable enhancement.
