Ok, about the only thing I would suggest you check, make sure that the 
Maildir was made BY that user, or it's oned by that user now.  Using the 
maildirmake command as root, in somebody elses directory isn't quite 
going to work.

Other than that, hope somebody else on the list can help.  Since I've 
never done PAM auth, this is beyond me.
