Have you run "makesmtpaccess" after editing smtpaccess ? Have you enabled ipv6? Check the maillog in order to see if the blocked addresses are noted as ipv6. If ipv6 is enabled you have to do your entries in smtpaccess also as ipv6. Or you rebuild courier without ipv6 support.
