Well I've just (re)read the INSTALL instructions and I don't see any mention of enabling or disabling it. Bizzarely, it has started working. I used the provided script to create a trial certificate for doing SSL for IMAP, SMTP & POP3 and all of a sudden ESMTP listed AUTH as a command. It wasn't just because I restarted esmptd either, as I'd done that many many times as I tried different things.
