I had a similar problem with SquirrelMail 1.4.0 and Courier 0.42.2. To
make it work, I had to make a change to the imap_general.php file (part of
SquirrelMail) on line 287:

Original (does not work):
  $query = 'LOGIN "' . quoteimap($username) .  '" "' . quoteimap($password) . '"';
My version (works):
  $query = "login \"$username\" \"$password\"\n";

I have not yet sent a bug report to the SquirrelMail folks. Not sure if
LOGIN or login really makes a difference, or if it is part of the
processing that the 'quoteimap' function performs.
