You don't really need to have a sample. Just open any doc already, or 
import a MS Word one. Or create a new one. Follow my steps and you're 
good to go.

Well, I'm not so sure about that. You might need the full version of 
Acrobat. Acroread won't let you save a file. You can enter them and 
internally the PDF has a URL to submit them to. But the form is R/O.
