I don't know if this applies, but FDF wraps the PDF. It is designed both 
for submitting back to a web link and for printing locally. The form 
fields on the original PDF have to be named. Then you can use Perl, PHP 
or other scripting language to "fill-in" values. I have such a script in 
Ruby. It fills in values from a form (Rails in this case) and streams 
the result back to the user. But just saving it to a file should also work.
