Most of the time this turns out to be something else.  The issue that you posted about was likely fully explained in my reply, but you are indicating that there is something else going on that is preventing Actions from being taken properly on messages.  It would help to post the logs associated with any message that didn't have a proper Action taken because there may be good hints in there.  Knowing your Declude and SmarterMail versions would also help.  For instance, there were some bugs in the initial 3.0 releases of SmarterMail that could have caused this, but I believe that they were fixed in the more recent release(s).

proper actions on the message, but if you are relying on things like 
Subject tagging and some sort of content filter past the Declude part, 
it will fail.
